 Q.  What Information does a nursing home need?

  A.  Our facilities ask that you bring the following information:

  • Financial Information

  • Medicare Card

  • Social Security Number

  • Referring doctor to follow in the nursing facility

  • Responsible party

  Families of those in need of care should have the following:

  • Power of Attorney

  • Advanced Directives

  • Living Will

  • Medicaid Card

    

  Q.  What about financial arrangements?

  A.  The decision to admit to a loved one to a long-term care facility is one of the most difficult a

        family must make. Financial arrangements are also a concern. TAG Management Services,

        understanding this, will help you explore your options of Private Pay, Private Insurance,
